This appointment is ideal for individuals needing specimen collection (tissue, blood, urine, etc.) for evaluation and/or diagnostic testing and have multiple requisition forms, are processed by more than one laboratory, require special handling, require the drawing of an excessive number of tubes, or you have a lab order AND kit draw (or any other service) to be performed. There may also be other special situations not mentioned above. A certified field technician will travel to your location and collect, prepare, and deliver specimens to the laboratory of your choice. Note: Please make sure we have received a signed Test Requisition Form (TRF) from the ordering physician or reference laboratory. TRFs can be faxed to (971) 777-7270, attention scheduling. A signed copy of your order must be received at least 24 hours before your appointment to avoid cancellation. Cancellations made up to 24 hours before a scheduled appointment via email, text, or phone call will be processed without a penalty. Alternatively, you can log into your VeniDirect account to make the necessary changes. Cancellations made 24 hours or less before an appointment will be subject to a charge of either 100% of reserved service or $50, whichever is greater. This includes appointments where our service provider is unable to access the property, is turned away, or the patient is unavailable at the specified time. In the event that VeniDirect cancels an appointment with less than 24 hours' notice, a new appointment will be scheduled for the patient without penalty. Availability permitting. VeniDirect provides irrevocable, non-tangible services, therefore a "convenience fee" is charged when you book. Except as outlined in our Cancellation Policy, the convenience fee is not refundable. Regardless of the circumstances, once a technician has been dispatched to your home or business, you are not eligible to receive a refund or credit.
